FEES & REGISTRATION

Fees:

Here is an explanation of how your fees are used.

1. Team/Player Registration.

2. Player, Coach, & Manager personal accident insurance.

3. Liability Insurance as required to protect the land owner (municipality), participant, and league.

4. League and Tournament Fees.

5. League T-shirt.

The current fees are $50.00 per player and $30.00 per coach and assistant coach. Make your check out to Smoky MTN. Soccer Program and mail your fees to:

A single check should be sent, along with the transmittal sheet, showing number of players and coaches registered, as well as other necessary information. Please do not send individual player checks as this can become very confusing and expensive if they do not clear. If you have any questions please call David Melendy at 679-0185. There can be additional fees due to the league if you:

1. Use US Cellular for practice or home games. The cost to you for each usage period is $20.00.
2. If you have Bob Bratta schedule referees for your home games through the season. This fee is $35.00 per team and is paid directly to him. A transmittal form accompanies your payment. This form available at sign-up meeting or can be downloaded below.

Registrations:

NOTICE: Due to late entries for the past several seasons we have been forced to send out abbreviated schedules and have had to make changes to the schedule after its initial printing. Teams have had to be moved from one division to another, forfeits issued, and many unhappy parents and coaches who cannot make the necessary arrangements for the games until the last minute. As a result in order to secure your team in the league, a minimum of $500.00 deposit must be submitted on or before the posted registration due date. A team may follow up with the balance and their roster within a 5 day grace period, but failure to submit all fees by this date will result in being omitted from the league.

You may register your team by downloading the registration MS Excel file below. (Note: please download the file; do not open it in your browser window). Once this file is completed, please send the completed file via email attachment to Alan Devan at adevan@smmss.net Remember to make sure that you include the t-shirt sizes of all players and coaches registered, and that the quantities match the numbers sent on the transmittal form. Managers do not receive a T- shirt unless ordered and paid for separately.

Waiver of Liability & Code of Sportsmanship:

The Waiver of Liability and Medical Release, and the Code of Sportsmanship and Behavior forms must be completed and signed before any player is authorized to practice or play in this league. You can download the forms below. The team manager or coach must keep the completed waiver forms and code of sportsmanship and behavior forms on hand at each game.
